Price at analysis: $367.07 (2026-08-27, post-market). 52-week range $328.57–$603.88 (stock is -39% from its all-time high of $616 and -21.8% over the trailing year). No prior report exists for this ticker; this is a from-scratch pipeline run.
0. Knowledge check
python .mcp/kb.py find ISRG returned two live notes, both read and applied:
- [[pattern-console-lock-in-is-symmetric]] — installed-console moats protect the incumbent for
short outages but exclude it for years once a rival console lands in the slot. Directly relevant:
ISRG is the incumbent now facing its first credentialed rivals in 20+ years.
- [[pitfall-vendor-forward-eps-is-the-wrong-fiscal-year]] — already carries an ISRG data point
from 2026-08-12 (vendor fwd P/E 33-34x on a FY2027 EPS, true FY-consistent multiple ~37-38.5x).
Reconfirmed below with fresh data.
No Themes/ sweep covers medtech/surgical robotics specifically — this is a fresh single-name look,
not a re-sweep.
1. General Health (Fundamentals)
Free cash flow — reconciled from quarterly statements, not the vendor snapshot aggregate
Guard applied: summed the four most recent quarters (Q3 2025–Q2 2026) two independent ways — each quarter's own pre-computed FCF cell, and OCF minus capex — and both agree to the dollar:
| Quarter | OCF | Capex | FCF |
|---|---|---|---|
| Q3 2025 | $841.0M | $105.4M | $735.6M |
| Q4 2025 | $892.5M | $162.5M | $730.0M |
| Q1 2026 | $911.9M | $103.3M | $808.6M |
| Q2 2026 | $1,061.0M | $112.6M | $948.4M |
| TTM sum | $3,706.4M | $483.8M | $3,222.6M |
The TTM OCF sum ties exactly to get_stock_info's operatingCashflow field ($3,706.4M). The
snapshot's freeCashflow field, however, reads $2,615.2M — 19% below the reconciled $3,222.6M.
This is the same defect [[pitfall-yahoo-snapshot-fcf-field-diverges-from-quarter-sum]] documented on
DPZ (also ~19% low, same direction). ISRG is a second confirming instance — see notes for the
orchestrator below.
- TTM FCF: $3.22B (reconciled) vs. calendar-FY2025 FCF of $2.49B (fin.py/roic.ai annual column, confirmed by both sources) — FCF accelerated sharply into 2026.
- FCF CAGR: annual FCF grew $958M (2022) → $750M (2023) → $1.30B (2024) → $2.49B (2025), a 3-year CAGR of ~37.5%, now running at $3.22B TTM. Longer 5-8yr history is not available from either vendor at the free tier ([[pitfall-roicai-free-plan-caps-history-at-two-years]] confirmed again — roic.ai returned only FY2024-FY2025). Treat the CAGR as directionally strong but short-window.
- TTM FCF/share: $3,222.6M / 358.3M shares ≈ $9.00.
Capital allocation
No debt (totalDebt: 0), no dividend (payoutRatio: 0). Capital allocation is 100% growth
reinvestment (R&D $1.31B in FY2025, +14% YoY) plus buybacks; acquisitions are trivial ($13.9M in
FY2025). TTM buybacks (sum of last 4 quarters) = $1,913.3M + $201.0M + $1,123.2M + $320.3M =
$3.56B — more than TTM FCF ($3.22B), funded from the $5.2B total cash/investments balance
(no debt anywhere in the structure). This is capital return running ahead of cash generation for a
stretch, not a warning sign given the balance sheet, but worth tracking if it persists.
Debt-to-assets
Zero. totalDebt: $0, current ratio 4.96x, quick ratio 3.64x. A fortress balance sheet — this
name carries no leverage risk under any scenario modeled below.
Shares outstanding trajectory
Diluted shares fell every quarter over the trailing year: 364.1M (Q2'25) → 361.8M (Q3'25) → 360.4M (Q4'25) → 359.8M (Q1'26) → 357.3M (Q2'26), a -1.9% YoY reduction. Buybacks now outpace SBC dilution — a tailwind to per-share value, reversing the roughly-flat 3yr share count fin.py's longer window shows (0.1% CAGR, i.e. dilution and buybacks offset over 2022-2025, buybacks now winning).
Top & bottom line growth
| 2025 | 2024 | 2023 | 2022 | |
|---|---|---|---|---|
| Revenue | $10.06B | $8.35B | $7.12B | $6.22B |
| Net income | $2.86B | $2.32B | $1.80B | $1.32B |
| Diluted EPS | $7.87 | $6.42 | $5.03 | $3.65 |
Revenue CAGR (3yr): 17.4%. Net income grew faster than revenue every year (operating leverage from the installed base plus the da Vinci 5 mix shift). TTM revenue growth 18.5% YoY (Yahoo), Q2 2026 revenue +19% YoY. Growth is real and margin-accretive, not bought — R&D/SG&A both grew slower than revenue in 2025.
Per-share metrics
Revenue/share $27.75 (annual) to $31.07 (TTM, Yahoo). FCF/share (TTM, reconciled) $9.00. Book value/share $51.34.
2. Moat & Competitive Advantage
Quantitative base
ROIC (roic.ai return_on_inv_capital): 13.63% (FY2024) → 14.75% (FY2025), rising. ROE 15.6% →
16.7%. Gross margin compressed slightly, 67.46% (2024) → 65.99% (2025) GAAP, but management raised
its 2026 non-GAAP gross margin guide to 68-69% (from 67.5-68.5%) at the Q2 2026 print —
management is confident margin is expanding, not eroding, even as competition arrives. Operating
margin is also expanding: 28.1% (FY2024 annual) → 29.3% (FY2025 annual) → 33.6% (TTM, Yahoo
snapshot) — margin trend supports "moat intact," not "moat eroding," on the numbers available
today.
Revenue-stream map
The da Vinci install base (11,710 systems at Q2 2026, +12% YoY) throws off recurring instruments/accessories and service revenue — the majority of total revenue and the highest-quality, highest-margin stream, priced per-procedure and largely insulated from any single quarter's system sale cycle. System (capital) sales are the smaller, lumpier stream, now increasingly leased rather than sold outright (management cited "a larger lease base" as a Q2 2026 revenue driver — a financing-engineering detail worth tracking but not yet a red flag). Ion (endoluminal biopsy) is a smaller, earlier-stage second platform extending the razor/blade model beyond surgery into diagnostics.
Adversarial stress-test — "how would a well-funded rival attack?"
This is no longer hypothetical. 2026 is the first year in Intuitive's 20+ year history that credentialed, FDA-authorized rivals exist in the same procedure categories: - Medtronic Hugo — FDA clearance for urologic procedures, December 2025. - J&J Ottava — FDA marketing authorization for soft-tissue general surgery (gastric bypass, gastrectomy, cholecystectomy, splenectomy, sleeve, bowel resection, appendectomy, hernia repair, and more), July 22, 2026 — five days after Intuitive's own guidance-driven selloff. - Smaller entrants (CMR Surgical, Distalmotion, Moon Surgical) are also live in the US.
One sourced metric captures the regulatory shift cleanly: "Contested Procedure Share" of US robotic surgery moved from 0% (Dec 2025) to 75.6% (July 22, 2026) as Hugo and Ottava authorizations opened urology and upper-abdomen general surgery to competition — i.e. three-quarters of the procedure landscape that was legally a da Vinci monopoly a year ago is now contestable in principle.
Applying [[pattern-console-lock-in-is-symmetric]] directly: the risk to ISRG is not the outage-type risk that pattern was written about — it is the mirror case. ISRG is the incumbent holding nearly every console slot today (11,710 installed systems vs. a handful of Hugo/Ottava placements just beginning). The pattern's mechanism says the switching cost that protects an incumbent from a short disruption is the same mechanism that will lock ISRG out of any OR where a rival console gets installed and surgeons complete their learning curve on it — for 3-5 years, the console's typical life. The number to track from here is not "contested procedure share" (a regulatory ceiling) but rival installed-base growth (the actual displacement rate) — which, as of this report, is still early and not yet disclosed in a way that lets this be sized.
Disruption forecast (5-10yr arc)
- Near-term (12-24mo): low risk of material share loss. Hospital capital budgets, surgeon credentialing, and Intuitive's ecosystem breadth (multi-port + single-port + Ion, 20+ years of published outcomes data) create real switching friction. Da Vinci 5 is still capturing the bulk of new placements (85% of US Q1 2026 placements were dV5 units) — Intuitive is not standing still while rivals ramp.
- Medium-term (3-7yr): the real test. If Hugo/Ottava execute well on price, workflow, or a differentiated angle (Ottava's arms-in-the-table design, lower capital cost positioning reported by some analysts), each console placement is durable share loss for the life of that unit. This is the scenario the framework should price some probability into, not dismiss.
- Long-term: minimally invasive robotic surgery as a category keeps growing (the tailwind is larger than the incumbent-vs-challenger question) — the debate is share of a growing pie, not whether the pie grows.
Evergreen assessment
Not a forever monopoly anymore — but still a forever category leader by installed base and switching-cost depth. The evergreen case rests on execution (continuing to out-place rivals on net, defending margin, extending the platform via Ion and software) rather than on the absence of competition, which was true until mid-2026 and is no longer.
Moat-defense evaluation
Management's response so far: raising 2026 gross-margin guidance despite competitive noise, holding full-year procedure-growth guidance (13.5-15.5%) rather than cutting it, and continuing to place da Vinci 5 units at an accelerating pace (246 dV5 placements in Q2 2026 vs. 180 a year earlier). This reads as confidence, not denial — but it is one data point against a rival that only received authorization five weeks before this report.
3. Valuation
No dividend (payoutRatio: 0, trailingAnnualDividendYield: 0) — DYT and DDM are N/A.
Graham and a growth/FCF-based approach carry the weight, cross-checked with Bogle for context.
Guard applied — vendor forward P/E is the wrong fiscal year, confirmed again
| field | value | what it actually is |
|---|---|---|
trailingPE |
42.19x | TTM (trailing 12mo EPS $8.70) |
epsCurrentYear / priceEpsCurrentYear |
$10.79 / 34.0x | FY2026 consensus — the true "current-year forward" multiple |
forwardEps / forwardPE |
$12.07 / 30.4x | FY2027 consensus — one fiscal year further out than it looks |
Exactly the mechanism [[pitfall-vendor-forward-eps-is-the-wrong-fiscal-year]] describes: the headline "forward P/E" (30.4x) looks cheaper than the true current-year multiple (34.0x) because it prices a later, larger EPS year. The correct current-year read is 34.0x, not 30.4x. This also refines the note's own 2026-08-12 ISRG entry (which had 33-34x vendor / ~37-38.5x true, at a higher price) — same direction, smaller gap now that the stock has de-rated.
Fair value range
- FCF multiple: TTM FCF/share $9.00 × 35-45x P/FCF (justified by zero debt, rising ROIC, no dividend drag) → $315-$405.
- Forward-EPS multiple: FY2027 consensus EPS $12.07 × 28-35x (a discount to ISRG's own historical 45-70x+ ttm bull-market band, reflecting the new competitive reality) → $338-$422.
- Graham IV
√(22.5 × 8.70 × 51.34)= $100.25 — far below spot. Expected and given almost no weight: Graham penalizes any company whose value is mostly future growth rather than current book value, and ISRG is exactly that company. Included for completeness, not as a constraint. - Bogle expected return (context, not a price target): 0% dividend + high-teens EPS/FCF growth consensus ± modest further multiple normalization (34x current-FY drifting toward a 28-32x steady-state as growth decelerates from the 13.5-15.5% procedure-growth guide) ≈ low-to-mid teens annualized expected return from here — a reasonable, not exceptional, forward return for new capital at $367.
Combined fair value range: $330-$410, centered close to the current $367 price. This reads as fairly priced, not clearly cheap — the ~40% drawdown from the all-time high has closed most, not all, of the valuation gap. Sell-side remains more bullish (mean target $478, median $484, 30 analysts, "Buy" consensus 1.82/5), but several targets were cut sharply on 2026-07-17 (Mizuho to $400, Evercore ISI to $375 — the lowest institutional target in the set) alongside the procedure- growth guidance disappointment, and one (HSBC) downgraded to Hold on 2026-07-28. Oppenheimer and UBS both upgraded in mid-to-late August, arguing the selloff overshot — a genuine two-sided debate among professional coverage, not a one-way read.
4. Type overlay
Mega-cap ($131.5B), non-dividend, debt-free growth medtech — none of the Dividend Grower / REIT / BDC / Small-Cap overlays apply. Standard growth-compounder framework, applied above in full.
5. Sentiment & Intelligence
- Q2 2026 (reported mid-July 2026): beat on both revenue and adjusted EPS ($2.80 vs. ~$2.51 consensus), but the stock sold off hard because US procedure growth moderated and management flagged tougher H2 comps plus a headwind from expiring enhanced ACA tax credits pushing some patients off insurance. Full-year procedure-growth guidance was held at 13.5-15.5%, not cut — the market reacted to the tone (deceleration flagged) more than a hard number change.
- China remains a stated soft spot, not quantified in the search results reviewed.
- Competitive news cadence has been unusually dense for this name in 2026: Medtrono Hugo clearance (Dec 2025), J&J Ottava FDA authorization (Jul 22, 2026) landing five days after the earnings-driven drawdown, and a wave of smaller entrants (CMR Surgical, Distalmotion, Moon Surgical) all surfacing in the same year — a genuine regime change in the competitive backdrop, not noise.
- Insider activity: dense but routine-looking. Every transaction reviewed (Aug 2025-Aug 2026, ~140 Form 4 rows) was either a scheduled option-exercise-and-sale or a small officer sale; no insider purchases appear in the window reviewed. CEO Rosa and Executive Chair Guthart both sold meaningfully ($4-17M individual transactions) at various points, consistent with 10b5-1 pre-set plans rather than a reaction to news (sales are spread evenly through the year, including at both the highs near $600 and the lows near $370-400). Not a red flag on its own, but no golden-flag insider buying to offset it either — normal for a mega-cap exec team, not a signal either way.
- Analyst target dispersion is wide ($324 low to $685 high on 30 analysts) — reflects genuine disagreement on how much the new competition matters, which mirrors this report's own tension between the Fundamentals/Moat read (still strong, still growing, margin still expanding) and the newly real competitive risk factor.
6. Synthesis & Verdict
Two questions, per the framework:
- Is this a good business? Yes, unambiguously by the numbers — debt-free, ROIC rising, gross and operating margins expanding even through a competitive-news-heavy year, revenue growing ~18-19% with net income growing faster, buybacks now shrinking the share count. The moat is weaker than it was twelve months ago (first real competition in 20+ years) but is not broken — installed-base switching costs are real and Intuitive is still winning the placement race today.
- Has the market priced that in? Largely, yes. The ~40% drawdown from the all-time high has pulled the current-FY multiple from an extreme (60-80x ttm at the highs) to 34x current-FY / 42x trailing — inside this report's computed fair-value range of $330-$410, not clearly below it.
Tension worth naming explicitly: Fundamentals and quantitative Moat metrics (ROIC, margins, placements) say the business is executing well through the competitive transition; Sentiment and the regulatory read (0% to 75.6% contested procedure share in one year) say the risk factor is new and unresolved, with the real test — rival installed-base growth — not yet visible in the data. Weighting: for a moat-dependent, non-dividend compounder at a still-premium multiple, Moat durability and Valuation should outweigh a single quarter's Sentiment reaction — the stock already priced in a great deal of the Q2 disappointment and the Ottava news together. But the competitive risk factor is real enough that it earns a named risk, not a footnote, and it argues against paying up for the name at spot.
Verdict: WATCH, conviction 6.5. A genuinely good business trading close to fair value rather than below it. The framework's conservative bias ("rather miss an opportunity than overpay") argues for waiting for a better entry — near or below the fair-value floor and below every sell-side target including the most bearish (Evercore ISI $375) — rather than initiating at $367. A drop into the $300-335 zone, or clear evidence that Intuitive is holding placement share against Hugo/Ottava through Q3-Q4 2026 prints, would upgrade this toward ACCUMULATE.
Key risks
- Competitive share loss is now a real, dated risk, not a hypothetical one. J&J Ottava (soft tissue) and Medtronic Hugo (urology) are both FDA-authorized and selling into US hospitals for the first time in Intuitive's history. Per [[pattern-console-lock-in-is-symmetric]], every console a rival lands is a 3-5 year lockout, and the disclosure needed to size this (rival installed-base growth) is not yet available.
- US procedure-growth deceleration flagged by management for H2 2026 (tougher comps, ACA subsidy expiry reducing insured patients) — a real, if modest, near-term growth headwind.
- Valuation is fair, not cheap — 34x current-FY earnings leaves limited room for multiple expansion even if the competitive risk does not materialize, and real room for further compression if it does.
- China softness, cited by management as ongoing, not yet quantified from sources reviewed here.
Data traps hit / confirmed this run
- [[pitfall-vendor-forward-eps-is-the-wrong-fiscal-year]] — fired again. Vendor
forwardPE30.4x prices FY2027 EPS; the true current-year (FY2026) multiple ispriceEpsCurrentYear34.0x. Used the current-year and next-year figures explicitly labeled throughout this report rather than quoting the bare "forward P/E." - [[pitfall-yahoo-snapshot-fcf-field-diverges-from-quarter-sum]] — fired a second time (first
was DPZ). Snapshot
freeCashflow($2,615.2M) reads 19% below the reconciled TTM quarter-sum ($3,222.6M), same magnitude and same direction as the DPZ case. This raises the pattern from a single instance to two confirmed instances on different tickers — worth the orchestrator upgrading that note's confidence frommediumtowardhighand adding ISRG to itstickerslist. - [[pitfall-roicai-free-plan-caps-history-at-two-years]] — confirmed again; roic.ai returned only FY2024-FY2025 for income statement, cash flow, and profitability ratios despite an 8-period request. Full 5-8yr CAGR work relied on fin.py's 4-year annual window plus quarterly data instead.
